Job Description:
A company is looking for a Licensed Care Coordinator.
Key Responsibilities:
Support patients in scheduling provider visits
Ensure proper documentation of all patient contacts
Support outreach and navigation teams with operational and
non-clinical tasks
Required Qualifications:
High School Diploma / GED (or higher)
Experience with escalated customer service issue resolution
Phone/Fax system experience
EMR experience, Athena & Epic preferred
