Job SummaryThe Unit Supervisor is responsible for the supervision of care management staff and the daily care management operations of the program. This includes direct supervision of staff, ensuring quality of care management services provided to clients, management of staff documentation requirements, and staff training. The supervisor will ensure the provision of adequate clinical support to clinically complex clients in crisis. The supervisor will report to the Clinical Director. Essential functions include:Direct supervision to care managers to assist with the development of effective clinical interventions and care plansEnsure staff is kept informed of Health Home policies and procedures as well as departmental priorities. Receive appropriate trainings in order to maintain clinical standardsMonitor Health Home compliance and quality assuranceProvide crisis intervention to clients in distressComplete enrollment and service date tracking reports monthly or as neededReview and approve documentation as assignedConsistently exhibit the values of team work, open book management, advocacy and customer serviceParticipate in quality assurance and quality improvement activitiesConduct staff meetings, trainings and supervisionExtensive field work is required to achieve client outcomes and meet health home requirementsPerforms other duties as requestedFacilitation of bi weekly Clinical Case Review with the Care ManagersWhat You NeedBachelor's Degree Required; Masters Degree Preferred in Social Work, Psychology or Human Services.Skills and/or Experience Required:Two (2) years post graduate experience working with individuals or families who have a history of mental illness, substance use, homelessness or HIV/AIDSExcellent problem-solving, decision-making and judgment skillsAbility to work on an interdisciplinary teamYou must have excellent interpersonal and time management skills.Strong leadership abilityPreferred:MSW/LMSW/LCSW2 years working with severe mental illness population1 year supervisory experienceby Jobble
