Job Description:
Carnegie Post Acute Skilled Nursing Facility, a leader in senior
rehabilitation and skilled nursing services, is seeking a dedicated
and compassionate Licensed Practical Nurse (LPN) to join our care
team. The ideal candidate will bring a mix of empathy, clinical
skills, and a passion for providing high-quality patient-centered
healthcare. This role involves direct patient care and
collaboration with a multidisciplinary team to deliver premium care
tailored to the needs of post-hospital and post-surgical seniors.
Administer medications and document therapeutic effects Monitor
patient health and update care plans Provide wound care and other
direct nursing care as prescribed Collaborate with healthcare
professionals to plan for patient care Assist with daily needs of
patients such as dressing, bathing and dining Educate patients and
families on health plans and home care Strong clinical skills
Excellent communication and interpersonal skills Proficient in
maintaining accurate records Ability to respond effectively to
emergencies Empathetic and compassionate patient care Knowledge of
infection control standards Medical Insurance Dental Insurance
Vision Insurance Valid Licensed Practical Nursing (LPN) license in
the state of employment CPR certification Preferred but not
required: IV certification, Wound care certification
