Job Description Job Description Position Title: Physical Therapist – Outpatient Location: Cortlandt Manor, NY 10567 Duration: 27-Week Contract Assignment Job Type: Contract | Local or Travel Schedule: 5x8.5-Hour Day Shifts (42.5 Hours/Week) Shift Details: Monday: 12:30 PM – 8:30 PM Tuesday – Thursday: 12:00 PM – 8:30 PM Friday: 9:00 AM – 3:00 PM No Weekends 30-minute break included Compensation: Local Pay: $54 – $60 per hour Travel Pay: $54 – $77 per hour Job Summary A leading healthcare facility in Cortlandt Manor, NY is seeking a licensed Physical Therapist (PT) for a 27-week outpatient assignment covering a full-time leave of absence. This role requires a strong outpatient background and the ability to work independently in a fast-paced clinical setting. The Physical Therapist will evaluate, plan, direct, and administer therapeutic treatments to restore function and prevent disability following injury, illness, or surgery. Key Responsibilities Perform comprehensive physical therapy evaluations Develop individualized treatment plans based on diagnosis and functional goals Administer therapeutic exercises, manual therapy, and modalities Monitor patient progress and adjust treatment plans as needed Complete timely documentation including evaluations, daily notes, progress reports, and discharge summaries Educate patients and families on treatment plans, home exercise programs, and injury prevention Collaborate with physicians and interdisciplinary teams to ensure coordinated care Maintain compliance with HIPAA, infection control, and facility policies Meet productivity standards and support quality improvement initiatives Utilize EPIC EMR for documentation Required Qualifications Active New York State Physical Therapist License (Required) Strong knowledge of outpatient rehabilitation practices Ability to work independently in a fast-paced outpatient environment Preferred Qualifications 1–2 years of outpatient physical therapy experience Experience treating orthopedic, post-surgical, and sports-related injuries Prior hospital-based outpatient experience Experience with EPIC EMR (Highly Preferred) Strong manual therapy and patient education skills For more details contact at pmohanty@navitashealth.com or Call / Text at 516-806-4227 .
