Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 days Shift: Mid Hours per Shift: 8 Experience: Two years of experience in a home setting within the last fifteen years License: Current RN Licensure in New York State Certifications: BLS (required before start date) Must-Have: - Recent experience conducting comprehensive assessments of patients Description: Urgent need for Registered Nurses in a home health setting. Candidates must have experience conducting in-home assessments. Responsibilities include evaluating health status, strengths, care needs, and preferences to develop individualized long-term care service plans. Candidates will review consumer medical documentation and health referral forms as relevant. They will enter evaluation data into an electronic form and transmit as required. Documentation of any concerns or conflicting information during the evaluation process is necessary. Continuity of care should be emphasized to reduce fragmentation and gaps in treatment plans. Candidates will discuss healthcare options, needed supports, and service vendor options with consumers. Initiation of communication between relevant parties for enrollment into managed care health plans is expected. Enhancing communication and collaborative relationships with interdisciplinary care team members is essential for improved care coordination. Collecting quality review data and required documentation to support outcome measurements is a key responsibility. Candidates must maintain a working knowledge of community resources and payor requirements for the target population. Meeting established standards for performance criteria is vital for this position.
