Medical Social Worker Location: White Plains, NY Agency: Cynet Health Pay: Competitive weekly pay (inquire for details) Shift Information: Days - 5 days x 8 hours Start Date: ASAP About the Position Job Title: MSW Care Manager Profession: Social Work Specialty: Care Management Duration: Part-time Shift: 9 AM - 5 PM, 2 Shifts per Month Hours per Shift: 8 Experience: 2 Years in Acute Care Hospital or Related Setting Preferred License: Licensed Social Worker (LMSW/LCSW) Certifications: NYS SCREEN Certification (to be obtained within initial 3 months of employment) Must-Have: Master of Social Work Degree from an Accredited School Description: The SW Care Manager performs the initial comprehensive assessment on admission in accordance with department policy. Screening all patients by utilizing established tools for high-risk indicators ensures that high-risk patient populations receive the appropriate supportive services for discharge to prevent readmission. The SW Care Manager assesses all populations for potential discharge planning needs. Ongoing reassessment of needs throughout the hospital course is provided. Understands and adheres to performance standards, policies, and behaviors. Completes a comprehensive initial assessment. Performs all activities for multidisciplinary care coordination at the intra-hospital and inter-hospital levels of care. Engages in all transitional planning activities, including patient/caregiver education, arrangement of all aftercare, and commercial payer involvement. Identifies high-utilizer patient root causes. Communicates and collaborates all relevant patient information with appropriate health team members. Ensures that department goals of length of stay reduction, readmission prevention, denial reduction, and satisfaction improvement are realized. Documents per department requirements.
