looking for an Assistant Case Manager to join our team! An ideal
candidate should meet the following requirements:Bachelor's Degree
in health or human services and one-year qualifying experience or,
Associate's Degree in health or human services or certification as
an R.N. or L.P.N. and two years of qualifying experience.
Experience with families preferred.A valid driver's license, clean
driving record and insurance card is required. Qualifying
experience: Verifiable full or part time case management or case
work with persons with HIV or AIDS, and/or persons with a history
of mental illness, homelessness, or chemical dependence.Our Company

(35 hours/week)Responsibilities:The Assistant Case Manager assists
the case manager with the placement of homeless clients with
housing and identifies permanent housing that clients can afford
independently in the community. Assists with linkages to
educational and/or vocational resources and budgeting skills to
support clients' financial independence. Provides engagement
efforts to engage landlords and retain consumers in housing. The
Assistant Case Manager assists with coordination and provision of
support services to clients of the housing program. Responsible for
the oversight of the housing sites including safety checks and
regular visits, rent payment, assisting in site maintenance, and
case management services or coordination with external Care
Coordinators or Case Managers, as appropriate. Implement contract
regulations and assist with preparation of necessary reports to
funding sources. Assist both Housing Specialist during client
intervention, participates in case conferences and assists with
maintenance of documentation/client files.Conduct outreach and
engagement efforts with landlords, brokers, realtors and
clientsIdentify and inspect rental units for potential
placementRetrieve community resources and donations for
clientsAssists with housing placement and securing necessary
benefitsWork with clients on developing budgets and ensure rent
paymentsProvide referrals, linkage and follow up to community
supports and programsCoach and encourage consumers in self-advocacy
and self-managementMeet with clients to discuss service plan
goalsProvide frequent contact, home visits and monitoring of
progressAccompany consumers on clinical and supportive service
appointmentsCoordinate services with Care Coordinator/Case
ManagerCrisis interventionParticipate in case conferencesAssist
with maintenance of binder/client files
